By completing this registration, I confirm I am the legal guardian to above named child and give permission for my child to have their photo taken during the Warchiefs Sports season. I agree to hold WARCHIEFS SPORTS, photographers, and its volunteers harmless from any and all liability. I give permission to WARCHIEFS SPORTS, and/or parties designated by WARCHIEFS SPORTS to photograph and videotape my child to be used in all forms of media for purposes of advertising, promoting and celebrating for no financial compensation. I hereby release WARCHIEFS SPORTS from any and all claims that may arise out of the use of the photographs, videotape and/or name of the person named. I understand there are no refunds unless the game is cancelled by the league. If the game is cancelled by the league and can not be rescheduled during the photography period, a full refund will be issued.
