By completing this registration, I confirm I am the legal guardian to above named child and give permission for my child to participate in All Star Tournaments hosted by Warchiefs Sports. I agree to hold Warchiefs Sports and its employees and volunteers harmless from any and all liability. I take full responsibility for my child while participating in any and all practices and/or games related to Warchiefs Sports. I give permission to Warchiefs Sports, and/or parties designated by Warchiefs Sports to photograph and videotape my child to be used in all forms of media for purposes of advertising, promoting and celebrating for no financial compensation. I hereby release Warchiefs Sports from any and all claims that may arise out of the use of the photographs, videotape and/or name of the person named. I acknowledge the risk to have contact with individuals who have been exposed to and/or have been diagnosed with any communicable disease including COVID-19. I knowingly and freely assume all risks, both known and unknown and assume full responsibility for my child's participation. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release and hold harmless Warchiefs Sports, their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsoring agencies, sponsors, advertisers, owners and lessors of the premises used to conduct the practices and games, with respect to all and injury, illness, disability, death, or loss of damage to person or property, whether arising from the negligence of the releasees or otherwise.
