  • By completing this registration, I confirm I am the head coach to above named team. I agree to hold Warchiefs Sports and its volunteers harmless from any and all liability. I take full responsibility for my team while participating in any and all practices and/or games related to Warchiefs Sports and agree to abide by the Warchiefs Sports Code of Conduct. I acknowledge the risk to have contact with individuals who have been exposed to and/or have been diagnosed with communicable disease including COVID-19 does exist. I knowingly and freely assume all risks, both known and unknown and assume full responsibility for my participation. I hereby release and hold harmless Warchiefs Youth Baseball, their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsoring agencies, sponsors, advertisers, owners and lessors of the premises used to conduct the practices and games, with respect to all and injury, illness, disability, death, or loss of damage to person or property, whether arising from the negligence of the releasees or otherwise.
