I take full responsibility for myself while practicing or playing games. I give permission to Warchiefs Sports, and/or parties designated by Warchiefs Sports to photograph and videotape me to be used in all forms of media for purposes of advertising, promoting and celebrating for no financial compensation. I hereby release Warchiefs Sports from any and all claims that may arise out of the use of the photographs, videotape and/or name of the person named. Volunteer adult coaches shall be responsible for adherence to the Warchiefs Code of Conduct. Volunteer adult coaches who violate Warchiefs Code of Conduct will be dismissed from coaching Warchiefs Sports and unable to return to any Warchiefs Sports Events. Volunteer adult coaches who violate Warchiefs Code of Conduct will be reported to the proper authorities for any illegal activities. VOLUNTEER ADULT COACHES: -will have a trustworthy relationship with youth players and provide a safe place for youth players to learn and grow. -will not use alcohol or drugs while working with youth players. -will maintain clear, appropriate, and professional boundaries with youth players. -will exercise discretion in having any physical contact with youth players. -will not engage in any type of physical, sexual, psychological, written and/or oral intimidation or harassment of youth players. -will hold other volunteer adult coaches accountable for maintaining Warchiefs Code of Conduct as well as high moral and ethical standards. They will take all reports of violations seriously and report any violation to the property authority following the reporting procedures provided. REPORTING PROCEDURES ALL instances of suspected violations to the Warchiefs Code of Conduct must be reported immediately to a Warchiefs Director. Warchiefs Sports strongly recommends the the following for instances of abuse and/or neglect: What To Do If You Suspect Child Abuse or Neglect: Call 855-4LA-KIDS (855-452-5437) 24 hours a day, 7 days a week to speak to a trained child welfare worker to report possible abuse and/or neglect. If known, provide: ▪ Name, address, age, sex, and race of child ▪ Nature, extent and cause of child’s injuries or endangered condition including any previously known or suspected abuse to the child or to the child’s siblings ▪ Name and address of child’s parent or caretaker ▪ Child’s family composition ▪ Names and ages of all other members of the child’s household ▪ Account of how this child came to the reporter’s attention ▪ Any explanation of the cause of the child’s injury or condition offered by the child, caretaker or any other person ▪ Any other information which the reporter believes might be important ▪ The report shall also name the person or persons who are thought to have caused or contributed to the child’s condition, if known, and the report shall contain the name of such person if he is named by the child. All Assistant Coaches will receive a coaching shirt.
